- Driver's License
- Birth Certificate or Social Security Card
- Current Physical (within 6 months of date of hire)
- TB test or Chest X ray
- Current CPR
- Proof of Auto Insurance
- Criminal Check (Allen County)
- Current RN/LPN license (if applying for nursing position)
- Current HHA/CNA certificate
